A patient who can say &#8220;Dr. Reyes restricted me to no lifting over ten pounds on March 4th&#8221; gets better coordinated care than one who says &#8220;I think someone told me to take it easy.&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The second is your claim, if you have one. Insurance adjusters look for gaps in treatment and inconsistencies in your story.
