This log helps you document something reminder apps were never built for: what your injury actually forced you to endure, month after month. Pharmacy records show what was prescribed, but they don’t capture the $40 you spent on ibuprofen and a wrist brace at CVS, the copay on every refill, or the fact that your muscle relaxer left you too foggy to safely drive to work. How to fill this medication log

Page 1 contains the main log, a table for prescription medications that tracks start and stop dates, medication name, dosage, frequency, prescribing doctor, the reason or injury it treats, and the cost per fill.

Below it sits a separate table for over-the-counter purchases, because pain relievers, sleep aids, creams, and braces you bought yourself are real injury expenses that don’t appear anywhere else on paper.

Page 2 tracks side effects. For each medication: what you experienced, how it affected your daily life, when it started, when you reported it to your doctor, and what the doctor did about it. There’s also a refill log at the bottom for dates and copay amounts, so it’s easy to total the running cost of your medications.

Why does the “reason” column matter so much?

Because an injury claim runs on connection. A list of medications proves you took medications. A list of medications tied to specific injuries proves your accident caused ongoing treatment. When the log reads “cyclobenzaprine, prescribed by Dr. Okafor, for lumbar strain from the accident,” the line does legal work. When it just reads “cyclobenzaprine,” it doesn’t.

The stop date matters for the same reason. How long you needed each medication tells the story of how long your injury lasted. A prescription that ran eight months says something a pharmacy printout of fill dates makes easy to miss.

Side effects are part of your claim

People treat side effects as an annoyance to push through. Legally, they’re more than that. Non-economic damages in a California injury case cover the full experience of what the injury put you through, and that includes what the treatment put you through. Nausea that ruined your appetite for a month. Drowsiness that kept you off the road. Sleep disruption that bled into your work. If a medication caused it and the injury caused the medication, it belongs in the record.

Report side effects to your prescribing doctor first, and write down the date you did. If a reaction is serious, the FDA’s MedWatch program accepts reports directly from patients. And a practical note that has nothing to do with claims: an accurate medication list is one of the most useful things you can hand any new provider. How to use our free medication log

Add a row the day a medication starts. When a dosage changes, treat it as a new row with a new start date so the progression is visible.

Save every receipt. Pharmacy receipts, drugstore receipts for OTC items, all of it. A folder or an envelope stapled to the log works fine.

Total your out-of-pocket costs monthly. It takes two minutes, and you’ll be surprised what it adds up to. Medical expenses are usually the anchor of an injury claim’s economic damages, and small recurring costs are the ones that slip through.

The tracker captures what happened at each visit, including new prescriptions; this log follows those prescriptions through their full life, from first fill to stop date. Between the two, your treatment history is documented from both directions.
